Case Management  is a process which assesses,
plans, implements, coordinates, monitors and
evaluates the options and services required to meet
health and social service needs..
Case Management  (Coordinacion de
Servicios) esta la evaluacion de necesidades, un
plan, y repasos de los planes, opciones y servicios
a satisfacer los necesidades de salud y servicios
sociales; por usando comunicacion y recursos
disponibles.
Doula - A Birth Doula is a trained birth
professional that provides physical and emotional
support  and education to a woman and her
partner during labor and delivery of the baby;
offering the use of comfort measures to ease birth.

Clinical studies have shown that having a Birth
Doula present at birth, often results in shorter
labor with fewer complications; reduces the need
for medications; reduces the need for use of forceps
or vaccum extractor; reduces the need for
C-section; and results in a more successful family
unit, with the parents having more self-confidence
and the mom having less post-partum depression.
Una Doula  (Ayudante Tradicional de Parto)- es
una asistente de parto profesional que provee el
apoyo fisico, emocionaly educacion a la madre y su
pareja durante el parto y nacimiento del bebe; por
usando maneras a relejarse como masaje y otra
cosas.

Los estudios clinicos demostraron que cuando una
Doula asiste con el parto y nacimiento, el parto
esta mas corto con menos complicaciones;
necesidades menos para medicamentos o el uso de
forceps; menos chanza necesitar el cesareo y tiene
la chanza mejororar la familia mas exito, porque los
padres sienten mas auto-confianza y la madre tiene
menos de tristeza post-parto.

PRENATAL CARE COORDINATION
BENEFIT

Prenatal Care Coordination services help a
member and when appropriate, the member's
family gain access to medical, social,
educational and other services related to the
member's pregnancy.  Wisconsin Medicaid
PNCC services are available to
Medicaid/BadgerCare eligible Pregnant
women with a high-risk pregnancy, regardless
of which HMO they belong to.  The risk is
determined using a specific criteria taking
many factors into account; and does not
necessarily mean that the woman's pregnancy
itself is currently high-risk.  It is a program of
prevention.
COORDINACTION DE CUIDADO
PRENATAL

Servicios de Coordinacion de Cuidado Prenatal
se ayuda una miembro y cuando apropriada la
familia del miembro obtener la ayuda medical,
social y educacional y otros servicios que son
relacionados al embarazo del miembro.  
Servicios de PNCC de Wisconsin Medicaid
estan disposnible de las mujeres calificadas por
Medicaid/BadgerCare y tienen un embarazo de
alto riesgo; y no importa cual programa de
HMO que ellos participan.   El riesgo esta
determinado usando muchos factores; y no
indiquen que el embarazo de la mujer esta en
riesgo ahora mismo.  Esta una programa de
prevencion.
PREMATURO

Es cuando un bebe se naczca temprano; antes que
37 semanas de gestacion..

Esta comun para los bebes se naczcan entre 38-42
semanas; pero los mas cerca de 40 semanas lo
mejor.

Los bebes  crecen los mas durante de las semanas
38-42.

Los bebes que se naczcan prematuro tiene el riesgo
a tener problemas de sus pulmones, infecciones, y
incapacidadas.
PREMATURE

A baby born before 37 weeks of gestation is
considered premature.

Most babies on average are born between 38-42
weeks; but the closest to 40 weeks the better for
the baby.

The time from 38-42 weeks is when babies grow
the most.

Babies that are born premature have a risk of
having problems with their lungs, infections and
disabilities.
LOW-BIRTH WEIGHT

Babies that are less than 5.5 lbs at birth are
considerd to be low-birth weight; and run a
greater risk of having complications.
BAJO PESO

Los bebes que pesan menos de 2500 gramos
estan bajo peso; y tienen mas riesgos de
complicaciones.
