Community Support Worker:

This position supports families enrolled in our programs, by providing
home visits; information regarding positive parenting techniques;
assistance with obtaining baby items, food, clothes, housing, medical
insurance, Quest card, etc.  Requires collaboration with other
community resources.

Qualifications for the Position:

  • Minimum of a high school diploma or GED; additional health or
    social service education or experience is a definite plus.
  • Additional training or education in Case Management is preferred
  • Bilingual English/Spanish for some positions

Skills and Knowledge Required:
  • Knowledge of pregnancy and child development
  • Parenting experience
  • Understanding of Cultural Competencies
  • Good Communication and Organizational Skills

Community Nurse Associate/Case Manager:

This position works with high-risk pregnant women in the community, by
performing initial and ongoing assessments of the woman's physical,  
psychosocial and environmental health needs; to prevent pre-term
labor and other pregnancy complications with special emphasis on
cultural differences and health disparities.

This position provides health and nutrition education for optimum
maternal and child health; and emcompasses the entire perinatal
period.  Home visits are required.

Qualifications for the Position:

  • Minimum of a Bachelors Degree in Nursing, Social Work or Human Services, with a
    valid RN, LPN or CNA credential and a minimum of 2-3 years experience in
    Community Nursing
  • Excellent organizational and communications skills essential
  • Specialized education in Pregnancy, Childbirth and the Post-partum period is
    required
  • Experience providing home visits is required
  • Experience in substance abuse issues and Teen Pregnancy highly desired
  • Spanish-speaking is a plus


Other Requirements:
  • Must have dependable, personal vehicle, valid driver's license
    and proof of minimum state insurance for home visits
  • Must be flexible and have a good sense of humor, and have good
    coping skills to deal with stress

These are part-time day, indepensent contract positions,
with the potential for full-time
