Community Health Model: 21st Century:

Focuses on the health maintenance of individuals in the
community; based on the interrelatedness of
physiological, psychological, sociocultural, spiritual,
developmental and environmental factors, with a specific
emphasis on sociocultural beliefs and practices; and the
interplay of psychological and sociocultural stress and its
effects on health.

This model also emphasizes a true cultural competence of
community health care providers, which transcends the
use of interpreters; and is a return to those traditional
practices that have been proven to be effective to
promote wellness. (Garcia, 2006)
